
The Jacksonville Jaguars Foundation and Baptist Health/Wolfson Children’s Hospital have partnered for the local implementation of the NFL’s Play 60 Challenge youth anti-obesity initiative to help prevent childhood obesity and promote healthy lifestyle choices by increasing physical activity and better nutrition.
Play 60 initiative has two focus areas: Community-based and School-based.
Community-based Component
Since 2007, Baptist Health and the Jaguars Foundation initiative has supported community agencies and after-school programs by providing more than $500,000 in grants to organizations that serve underprivileged and disadvantaged youth including: Community Connections at the A. L. Lewis Center, Girls on the Run of Northeast Florida, Metro Kids Konnections at Cleveland Arms Apartments, PACE Center for Girls-Jacksonville; Gateway Community; and First Coast Kids Triathlon.
The Jaguars Foundation and Baptist Health/Wolfson Children’s Hospital are now accepting letters of query from agencies wishing to be a part of the NFL Play 60 Challenge. Interested agencies must submit their letters of query via the online form location at http://www.jaguars.com/foundation-community by midnight, January 15, 2013.
School-based Component
The Play 60 Challenge Initiative has partnered with Duval County Public Schools to engage sixth grade students to become more physically active and make better eating choices. During the 2012- 2013 school year, Baptist Health and the Jaguars Foundation will support 18 area middle or K-8 schools through the NFL Play 60 initiative.
Due to significant state and local budget cuts in education, schools face the challenge of supporting core academics to promote student advancement, while at the same time trying to sustain courses and activities that help promote a healthy life style and overall well being. Play 60 has provided schools with an avenue to incorporate 30 minutes of physical activity within the context of a school day; as well as challenging students to integrate 30 minutes of physical activity after school.
Each school will receive a $1,500 grant. This will allow participating schools to purchase equipment such as pedometers, exercise bicycles, pull-up bars, balls for various sports and exercise activities, speed ladders, jump ropes, water bottles, and pulse heart rate monitors. They will also able to purchase items to use as student incentives. This year participating Play 60 schools will attend the Jacksonville Jaguars home game against the Tennessee Titians on Sunday, November 25th, at 1:00pm.

The Jacksonville Jaguars Foundation and Wolfson Children’s Hospital are awarding up to $150,000 in grants to local nonprofits. Please carefully review the information and deadlines below.
DEADLINES:
Initial Letter of Inquiry due before January 15, 2013.
Application deadline is February 15, 2013.
WHO CAN APPLY?
Wolfson Children’s Hospital and the Jaguars Foundation only fund 501(c) 3 nonprofit programs in the five county area (Duval, Clay, St. John’s, Baker and Nassau) that serve economically and socially disadvantaged youth. We generally do not provide grants directly to schools; grants for religious purposes or that has a religious requirement; grants to “single disease” organizations; grants to “single sport” organizations; grants for events or one-time activities; grants for clubs or teams to go to tournaments; or grants to individuals.
There will be an on-line eligibility quiz before proceeding to the on-line application process.
HOW TO APPLY:
Click on the ‘PROCEED TO ON-LINE APPLCATION PROCESS’ link below. If you are a new applicant to this system, you will be asked to create an account for your organization. This account allows you to access your saved and submitted applications at any time. It also allows us to send you a submission confirmation e-mail and notify you if additional information is necessary to process your application.
Login to the online application using the email and password. You will be asked to complete an eligibility quiz before proceeding to the Letter of Inquiry. In your Letter of Inquiry, please specify that you are applying for a NFL Play 60 Grant. The deadline to submit a Letter of Inquiry for a NFL Play 60 grant is, January 15, 2013. We strongly advise that you submit your letter of inquiry well before this date.
After receiving your completed Letter of Inquiry, the Jaguars Foundation and Wolfson Children’s Hospital will review your initial proposal and make a determination if you may proceed to complete a full proposal. We will inform you of our decision, within 30 business days. If you are asked to proceed, log back into the applicant system and you will then see the link to access the Full Proposal. The deadline for Full NFL Play 60 Proposal is February 15, 2013.
